At Health Integration Partners (HIP), our Primary Care team works solely with NHS general practice and Primary Care Networks. We are clinicians and primary care leaders first, consultants second.

We know how hard primary care is right now. We understand the pressure of rising demand, workforce shortages, regulation, and constant change — because we have worked in practices and PCNs ourselves.

Our role is to stand alongside teams, help make sense of complexity, and support real, achievable improvement.

Our Primary Care team includes experienced GPs, practice partners, managers and PCN leaders who have led services through challenge and change.

  • A 100% success rate in CQC improvement, including supporting practices rated Inadequate to achieve a Goodrating within a year

  • Hands-on resilience support for over 250 practices, from small, single-site practices to large organisations

  • Support for more than 200 PCNs nationally, from early formation through to mature, system-facing networks

Our services

Practical support for practices and PCNs — when and where it’s needed

We offer a wide range of primary care support, but our approach is always the same: listen first, understand the problem, and work with you to find solutions that are realistic and sustainable.

Our services are flexible and can be tailored to support individual practices, groups of practices, PCNs or systems.

  • We support practices at every stage of the CQC journey — from preparation and recovery through to embedding lasting improvement.

    This includes:

    • CQC inspection preparation and mock inspections

    • Support for practices rated Requires Improvement or Inadequate

    • Quality improvement planning and delivery

    • Governance, policies and evidence frameworks

    • Ongoing support to maintain standards after inspection

    Our focus is not just on passing an inspection, but on helping teams feel confident, safe and well-prepared.

  • We help practices and PCNs stay resilient during periods of pressure, change or uncertainty.

    This includes support with:

    • Workload and access pressures

    • Workforce challenges and role optimisation

    • Operational flow and demand management

    • Partnership and leadership support

    • Short-term stabilisation and longer-term recovery

    Our work is practical, hands-on and designed to reduce pressure rather than add to it.

  • We support PCNs at every stage of their journey — from early development through to mature, system-facing organisations.

    This includes:

    • PCN set-up and governance

    • DES delivery and optimisation

    • Role development and workforce planning

    • Financial and contractual support

    • Supporting PCNs to work effectively within systems and neighbourhoods

    We help PCNs move from “getting by” to working with confidence and clarity.

  • Change in primary care can be difficult — especially when teams are already stretched. We help make change manageable.

    Our support includes:

    • Service redesign and pathway improvement

    • Change planning and implementation

    • Programme and project management

    • Supporting teams through transition and uncertainty

    • Aligning change with clinical safety and quality

    We focus on change that makes day-to-day work easier, not harder.

  • Not all support fits into neat categories. We also provide:

    • On-site and remote hands-on support
