We Understand General Practice

At Health Integration Partners (HIP), our Primary Care team works solely with NHS general practice and Primary Care Networks. We are clinicians and primary care leaders first, consultants second.

Practice and PCN Resilience

We help practices and PCNs stay resilient during periods of pressure, change or uncertainty.

PCN Development and Maturity

We support PCNs at every stage of their journey — from early development through to mature, system-facing organisations.

This includes:

  • PCN set-up and governance

  • DES delivery and optimisation

  • Role development and workforce planning

  • Financial and contractual support

  • Supporting PCNs to work effectively within systems and neighbourhoods

We help PCNs move from “getting by” to working with confidence and clarity.

Change Management and Transformation

Change in primary care can be difficult — especially when teams are already stretched. We help make change manageable.

  • Service redesign and pathway improvement
